The preview profile looks nothing like end result applied.

Everything works fine in constructing the profile and tweaking noise reduction settings, etc. but when the effect is applied, it appears to be incredibly smooth and plastic. So much so, that even if noise reduction is set to 0 across the board, the resulting applied effect will still be overly smooth. It does seem to vary with settings, but it's completely unmanageable to get a reasonable effect.

I attempted it in Premiere CC on an MXF clip.

If the results look too smooth and plastic then the profile may be inaccurate. Did you make sure no details were present in the analysis area used by Auto Profile?

Also, the preview inside Neat Video window shows the results of the spatial filter only, while the cumulative result of both temporal and spatial filter is presented in Premiere preview. That cumulative result normally shows less noise than the preview in Neat Video window because of additional noise reduction by the temporal filter. Try to disable the temporal filter (set the radius to 0), this should make previews in Premiere and Neat Video window equal.
